SOUHRN
Transplantace ledviny je formou léčby selhání ledvin, která na rozdíl od eliminačních metod, řeší kauzálně základní onemocnění pacienta, protože zcela nahrazuje funkce vlastních ledvin. Příprava k transplantaci, samotný chirurgický výkon i následná léčba jsou poměrně dlouhý a náročný proces. Každý nemocný by měl být důkladně informován ještě před zařazením do čekací listiny tzv. waiting listu o kladech a záporech transplantace. Předpokladem úspěšné transplantace je spolupráce pacienta a multidisciplinárního zdravot-nického týmu. Autorky charakterizují transplantaci ledviny, základní charakteristiky přípravy pacienta k transplantaci a specifika pooperační ošetřovatelské péče o pacienta po transplantaci ledviny. Uvádí návrh standardu ošetřovatelské péče - Pooperační péče o pacienta po transplantaci ledviny.

SUMMARY
In contrast with the eliminations methods, kidney transplantation is a form of treatment of renal failure which solves the cause of the underlying disease of the patient by complete replacing the function of the kidneys. The preparation for transplantation, surgical intervention alone and subsequent treatment are a part of a rather long and demanding process. Before being put on a waiting list, every patient should be informed in detail about the advantages and disadvantages of the transplantation .Good cooperation between the patient and multidisciplinary health care team is a prerequisite of successful transplantation. The authors characterize the kidney transplantation, basic characteristics of the patient´s preparation for the transplantation and specificities of post-operative nursing care for such patients. They also submit the proposal of the standard of nursing care - post-operative care for the patient after kidney transplantation.
